The Family Health Center (FHC) provides continuous comprehensive health care for the entire family, from the newborn to the elderly, in the ambulatory office setting. Our services include: the management of acute and chronic diseases, routine physical exams, health maintenance, wellness promotion, women’s health, low risk obstetrical care, and minor office procedures. Our physicians and staff serve as the patient’s or family’s advocate in all health related matters, including the appropriate use of consultants and community resources.

    • To provide continuity of care, the staff of Family Health Center will try to schedule you with your assigned physician.

To schedule an appointment, please call 937-734-4141

If you will be more than 15 minutes late to your appointment, please call the center to verify that you can still be seen.

If you cannot keep your appointment, please call at least 24 hours in advance in order to open up that time for other patients.

Care is provided by physicians from Wright State Physicians Inc. and FRHC Physicians, Physician Assistant and Certified Nurse Practitioners.  We mentor students from the Wright State’s Boonshoft School of Medicine as well as Nurse Practitioner Programs.
