When the new Greene County Public Health building is completed in early 2018, Five Rivers Greene County Health Center will also move into the new building which will have the same address.

This health center is a Health Center Program grante under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). FRHC is protected by the Federal Tort Claim Act Program (FTCA).

On April 10, 2017 Five Rivers Health Centers expanded with the opening of the Five Rivers Greene County Health Center at 360 Wilson Dr., Xenia, Ohio 45385.

The new health center will provide comprehensive medical, dental and behavioral health care services to all members of your family.  The Center is located within Greene County Public Health (GCPH).

Services Offered

  • Routine physical exams for children and adults, sports physicals, vaccinations
  • Management of acute and chronic diseases (sick or ill visits)
  • Routine prenatal care, well-women annual exams and appointments
  • Care of sexually transmitted diseases, birth control education and services
  • Complete dental care including teeth cleaning, fillings, extractions
  • Behavioral health services

If you do not currently have a medical home, the new Five Rivers Greene County Health Center would love to become the medical home for you and your entire family, including newborns to Senior Citizens. Five Rivers Greene County Health Center accepts all insurances including most Medicaid insurances (except Molina) and offers a sliding fee scale for patients without insurance.

To call and schedule an appointment, call 937-708-3400 M-F 8am-4:30pm

 

Bring the following to your appointment:

*Any medications you are taking other than vitamins or iron supplements

*Any questions you have (preferably written on paper).

If you will be more than 15 minutes late to your appointment, please call the center to verify that you can still be seen.

If you cannot keep your appointment, please call at least 24 hours in advance in order to open up that time for other patients.

 

Insurance and Sliding Fee Scale Information        

 *All co-pays or office visit fees are due at the time of service.

*To qualify for sliding fee scale you must be an uninsured Ohio resident with total household income not exceeding 200% of the Federal Poverty Guidelines. Proof of household income is required, such as income tax return, copies of your last (2) pay stubs, W-2 or proof of government assistance. Program details and an application can be obtained from the registration staff.
