Pediatric Center

We are pleased to announce on January 7th, 2013 Wright State Pediatrics moved their Pediatrics practice to Five Rivers Health Centers- Pediatrics. Conveniently located at the corner of Brown and Wyoming Streets–adjacent to the Center for Women’s Health location.

Free parking is available in front of the building.

Insurance and Sliding Fee Scale Information

*All co-pays or office visit fees are due at the time of service.

*To qualify for sliding fee scale, you must be an uninsured Ohio resident with total household income not exceeding 200% of the Federal Poverty Guidelines. Proof of household income is required, such as income tax return, copies of your last pay stubs, W-2 or proof of government assistance. Program detail and an application can be obtained from the registration staff.

*Care is provided by Physicians and Certified Nurse Practitioner from Wright State Physicians Inc. and FRHC Physician and Physician Assistant.  We mentor students from the Wright State’s Boonshoft School of Medicine as well as Nurse Practitioner Program.
